The Y provides NDIS support coordination services to support you in navigating your way through the NDIS world.
If you are already NDIS-approved, we can help you find the right providers with the right services to meet your personal needs.
We work with you to put your NDIS plan into action. We are a trusted advisor who can help you coordinate a range of supports, both funded and mainstream, to ensure you are getting the right services.
Working closely with you and your support network, we help you:
- make progress with your goals and aspirations
- explore your options for different service providers
- develop resilience in your network and community.
Our support coordination service strengthens your ability to self-manage your support under the NDIS.
We also help families with children in our early learning services who may be eligible to receive early intervention or NDIS support, to connect with the NDIS. Our aim is reduce the time it takes for acceptance into the NDIS and approval of the initial NDIS plan.
