Understanding the NDIS journey with The Y Whittlesea

Understanding the NDIS journey with The Y Whittlesea

The National Disability Insurance Scheme (NDIS) is an Australian government-funded program that provides support to individuals with disabilities. The NDIS encompasses a wide range of disability-related supports which organisations like The Y, play a crucial role in providing.

Inclusion Services by the Y offers NDIS support coordination, guiding participants through the complexities of the scheme and helping them access the right services to meet their specific requirements.  Our Support Coordinators help implement NDIS plans, support you in finding the right providers with the right services to meet your personal needs. We help you coordinate funded and mainstream supports to ensure you receive the right services.

Support Coordination is a funded support which will need to be requested in your NDIS plan.

Here’s a comprehensive guide to navigating this journey:

1. Eligibility Criteria for NDIS

The initial step in your NDIS journey is determining your eligibility. To qualify for NDIS support, you must be an Australian citizen or permanent resident, have a permanent disability significantly impacting your daily life, and be under the age of 65.

2. Accessing the NDIS

Once eligibility is established, you can begin the process of applying for the NDIS and exploring inclusive programs offered by The Y and other NDIS service providers. Start by completing a NDIS Access Request form, providing details about your disability and your support needs.

3. Assessment for Tailored Support

This step involves a comprehensive assessment led by a NDIS representative. This evaluation aims to understand your disability and support requirements, including your goals and aspirations, making sure to incorporate inclusive programs that align with your needs.

4. Personalised NDIS Plan

After the assessment, you’ll receive your personalised NDIS plan. This document outlines the supports you’ll receive, encompassing specialised services. Your plan is designed to address your unique needs and objectives.

5. Provider Selection for Inclusive Support Services

With your NDIS plan in hand, you have the autonomy to select service providers who will implement the supports identified in your plan. The Y can help you explore your options from a diverse range of providers offering specialised services. These options might include 1:1 home-based support, inclusive social groups, school holiday programs and camps.

6. Regular Plan Review and Inclusive Community Engagement

Your NDIS plan is dynamic, subject to regular reviews to ensure it evolves with your changing needs and goals. These reviews may occur annually or more frequently if necessary, facilitating continued participation in 1:1 support or inclusive community programs and social engagement.

7. Implementing Supports with NDIS Support Coordination

The final stage of your NDIS journey involves putting your plan into action. You’ll collaborate with your chosen providers to implement the supports, ensuring you benefit from services.

Throughout your NDIS journey, active participation in decision-making, goal-setting, and managing your chosen supports is encouraged. The Y Whittlesea has Support coordination available to assist you in navigating the NDIS system effectively, maximizing the opportunities and services available to you.
