Person-centred Disability Support

Why individualised support matters for your family

When you’re choosing disability support for your child or family member, you want more than a service — you want support that truly understands them.

That’s the heart of person-centred disability support.

At Y Whittlesea, we know that every child, young person and adult we support has their own strengths, interests, challenges and dreams. Individualised support ensures your family member isn’t expected to fit into a program — instead, the program is shaped around them.

What person-centred support means for your child

Person-centred support begins with listening.

We take time to understand:

  • What does your child enjoy?

  • What kind of Support Worker will be the best fit for your child?

  • What makes them feel confident?

  • What are their goals — big or small?

  • What would greater independence look like for your family?

From there, support is tailored to suit their personality, comfort level and pace. Whether that means starting slowly in a small group, building confidence through 1:1 Support, or exploring new community experiences, every step is guided by what feels right for them.

As one parent recently shared:

“For the first time, my son isn’t being asked to fit in. The program fits him — and that’s made all the difference to his confidence.”

Building confidence, independence and belonging

Individualised disability support helps children and adults to:

  • Build independence in everyday life

  • Develop social skills and friendships

  • Try new experiences in a safe environment

  • Strengthen communication and life skills

  • Feel valued and included in their community

Another participant told us:

“I like that they ask me what I want to do. I get to choose. It makes me feel grown up.”

Choice and voice are powerful. When people feel heard and respected, confidence grows — and that confidence carries into school, home life, friendships and the wider community.

Y Whittlesea Disability and Inclusion Programs

Our person-centred supports include:

Each program is flexible and can be adapted to suit individual needs, goals and family preferences.

Support that grows with your family

Life changes — and support should evolve too.

We work closely with families, carers and support networks to regularly review goals and adapt support as needs change. Our approach is collaborative, respectful and strengths-based.

At Y Whittlesea, person-centred support reflects our commitment to community wellbeing and creating a world where every person can thrive. Because when support is built around your child’s strengths, interests and aspirations — not around a fixed model — growth happens naturally. And when your child thrives, your whole family feels it.
