Navigate any problems, changes, or barriers that come up.
Work together with your family, carers, and other professionals to get the best outcomes.
Support Coordination is all about building your skills, not taking over. You stay in control of your choices, while your Support Coordinator helps you explore options, understand your rights, and take the next step toward your goals.
