Vaccination centre for people living with a disability 

A vaccination centre specifically for people living with a disability, and staff who care for them in their homes, is now open.

Eligibility:  

• People living within supported accommodation and the support staff for those facilities 
• If there are 2 or more people living with a disability at a single residence, they and all other people living within that household

Venue: Aspen Medical

Location: 258 Settlement Road, Thomastown

Open: Monday to Friday, 8:30am to 4pm

 Staff must bring the following to their appointment:

• A completed COVID-19 Vaccination Consent form, available at  https://www.health.gov.au/resources/publications/covid-19-vaccination-consent-form-for-covid-19-vaccination
• A completed copy of the attached form titled “COVID-19 VACCINATION eligibility declaration form”
• Photo ID
• Medicare Card
